At 62, twin sisters Rose and Betsy are an integral part of their small Midwestern community. When uninsured Rose is diagnosed with advanced stomach cancer, Betsy compels her to move to Florida, swap identities and fool the system, so Rose can access Betsy’s health insurance and—just maybe—save her life.

Kaki, a mythical beast from the Luo’s oral tradition, intertwines the struggles of two women on opposite sides of the world: a Luo AIDS widow in Kenya struggling to survive tuberculosis together with her daughter while gaining independence from her late husband’s family, and a Latinx scientist in the US whose findings on tuberculosis and HIV cause her career to come into conflict with the development of a novel HIV drug.
